Rocky Mountain Care of Logan is pleased to announce an opportunity for a full time Occupational Therapist working between our SNF/Home Health patients based out of our beautiful Logan, UT location. Our OT will enjoy a $5,000 signing bonus. Our OT will be compensated hourly as well as $55-$65 per home health visit.We're excited to offer our therapists an opportunity to deliver patient centered care. We empower therapists to decide what their patients need, and allow enough time in their day to deliver. There are no 15 minute encounters, we do not schedule at 125% production, and we encourage our therapists to spend the time to ensure each patient receives the care they require. Responsibilities Address client's aspects of performance to support engagement in occupations that affect health, well-being and life quality Select and adapt the appropriate per case occupational therapy treatment model, method and approach to direct the process of interventions Apply interventions to maximize safety and performance in activities of daily living (ADL) and instrumental activities of daily living (IADL) Teach clients new ways of approaching tasks and educate them on how to break down activities into achievable components Assess client's home and other environments and advice on alterations consistent with their needs and capabilities Recommend adaptive equipment and train in its use Guide and educate family members and caregivers Observe, note, and report on progress of long and short term treatment goals Collect data and document processes followed Skills Proven working experience as occupational therapist Excellent knowledge of principles and practices of occupational therapy Hands-on experience with computer programs and creative media Familiarity with ADLs evaluation tools ( e.g. Katz ADL scale, Lawton IADL scale) Ability to manage patients with different types of personalities Current knowledge of treatment practices per discipline and drive to continually learn and grow in the profession Professional behavior Excellent communication skills Valid license to practice occupational therapy in Utah Reliable vehicle, current auto insurance, and valid driver's licenseOur Occupational Therapist will join in our goal to help each resident restore their quality of life and get back to living their lives to the fullest extent possible. Our OT will be seeing patients between our SNF and our Home Health division.
